ABORDAGEM ANESTÉSICA NUMA DOENTE COM ESTENOSE AÓRTICA GRAVE E FRACTURA DO COLO DO FÉMUR | ANESTHETIC MANAGEMENT FOR FEMORAL NECK FRACTURE SURGERY IN PATIENT WITH SEVERE AORTIC STENOSIS

Resumo


RESUMO
Descrevemos a conduta anestésica que tomámos numa doente de 87 anos com estenose aórtica grave proposta para redução
e osteossíntese, com Dynamic Hip Screw, de fractura transtrocantérica do colo do fémur. Em virtude das limitações anestésicas
inerentes a esta patologia, e como forma de obter a maior estabilidade hemodinâmica possível, optou-se por realizar um blo-
queio combinado de três nervos periféricos do membro inferior (femurocutâneo lateral da coxa, femoral e ciático) associado
a uma sedação ligeira. Dada a elevada morbimortalidade perioperatória desta patologia, esta técnica permitiu, por um lado,
uma anestesia eficaz, e por outro, alterações cardiovasculares mínimas, pelo que achamos relevante relatar este caso.

ABSTRACT
Describing the anesthetic procedure followed in a 87 year old female patient with severe aortic stenosis elected for transtrochanteric femoral
neck fracture osteosynthesis and reduction with Dynamic Hip Screw. In virtue of the anesthetic limitations inherent to this pathology, and in
order to achieve the greatest hemodynamic stability possible, it was elected to perform a combined block of three lower limb peripheral nerves
(Femoral, Ciatic and Lateral Cutaneous Nerve of the Thigh).
